Study Information & Consent Forms
Are you?
- A young woman between 16-24 years of age
- Pregnant or up to 2 years postpartum
- Do you smoke NOW or did you smoke BEFORE you were pregnant
If you answer yes to these questions, you are eligible to participate in our study!!
We Want to Know:
- The factors that influence your smoking behaviour
- Your attitudes towards and experiences with local 'quit smoking' services offered before, during and after pregnancy
What We Need From You:
- Your current contact information
- A signed consent to join the study
-A scheduled time to complete an interview with you in-person or over the phone
You Will Recieve:
-As a thank you for your participation in the study,
you will recieve your choice of a FREE grocery or
Shopper's Drug Mart gift card
- If you complete one interview, your gift card value will be $15.00. If you complete a second, shorter interview you will recieve a total of $20.00
